This live webinar will feature a detailed overview of how to submit 2021 data online to be recognized in our 2022 Recognition Programs for Target: BP, Check. Change. Control. Cholesterol, and Target: Type 2 Diabetes. This will also cover how to attest to the evidence-based BP activities to be eligible for Target: BP award levels, Silver and Gold+.
