Award Achievement
Target: BP recognizes health care organizations annually with achievement awards by celebrating those that commit to improving blood pressure (BP) control, attest to evidence-based practices, and achieve BP control rates of 70 percent or more with the patients they serve.
Levels of achievement
Practices are recognized for attaining various levels of achievement.
Submit data and report ≥70% BP control among patients served with hypertension and attest to at least 4 of 6 evidence-based criteria in the Measure Accurately Pillar, the Act Rapidly Pillar, and one additional Pillar.
Submit data and report ≥70% BP control among patients served with hypertension.
Submit data and attest to at least 4 of 6 evidence-based criteria in the Measure Accurately Pillar, the Act Rapidly Pillar and one additional Pillar.
Submit data and attest to at least 4 of 6 evidence-based practices in the Measure Accurately Pillar.
Submit data for the first-time and commit to reducing the number of adult patients with uncontrolled BP.
