Data Platform Resources

Organizations participating in the Target: BP initiative gain access to the data platform (aha.infosarioregistry.com). Here, they can utilize benchmarking reports to support their quality improvement activities and submit data annually to earn achievement award. 

The first step to gaining access to the data platform is to Register Online. To register, you will need to know the following:

  • Your organization’s contact information
  • Your organization’s total adult (18-85 years) patient population count
  • Your organization’s total adult patients (18-85 years) that are a race other than white and/or identify as Hispanic or Latino ethnicity
  • The total number of practice, health center, or clinic locations in your health system.
  • Your organization’s characteristics, such as multi-specialty, Federally Qualified Health Center, etc.

Familiarize yourself with the data platform by utilizing our Navigating the Data Platform PDF Guide and Demo Video.  These resources will help you gain access and set up your site characteristics to make the most of our operational reports that allow comparison to other similar organizations.

If your organization is interested in receiving a Target: BP Achievement Award, you will need to submit aggregate data from the prior calendar year between January 1 and the May deadline. The annual program form questions will include:

  • Total adult patient population
  • Total adult patient population breakdown by race and ethnicity
  • Total adult patient population breakdown by payor group
  • Total number of adult patients with hypertension*
  • Total number of adult patients with controlled hypertension*
  • Attestation questions (yes / no / unsure) regarding evidence-based activities at your organization

*with inclusion / exclusion criteria defined by MIPS #236: Controlling High Blood Pressure.

The Data Platform has two Target: BP Operational Reports that contain visualizations on your organization’s control rate and attestations:

  • Target: BP Report (NQF18/MIPS#236) – This report displays the percentage of patients 18 to 85 years of age who had a diagnosis of hypertension (HTN) and whose blood pressure (BP) was adequately controlled (<140/<90 mmHg) during the measurement year.
  • Target: BP Evidence-Based Activities Report – This report displays the number of evidence-based activities to which you attested in each of the five pillars (Measure Accurately, Act Rapidly, Partner with Patients, Self-Measured Blood Pressure [SMBP], and Equitable Health Outcomes) during the measurement year.

Both reports offer the option of selecting different parameters (including time periods, state, etc.), and additional filters (such as FQHC, Rural, etc.) become available based on your Site Characteristics questionnaire under Form Management. Annual data submission provides an opportunity to evaluate your performance and identify or refine your blood pressure control quality improvement goals. The American Heart Association and American Medical Association recognize that health care organizations have dynamic needs, preferences, and capacities to prioritize and focus on hypertension control. Learn more about additional QI support options.
