Partnering with Patients – Step 3 in Hypertension Control Webinar – Lifestyle modification (Continuing Education activity)

When partnering with patients the goal is to empower patients to control their blood pressure. This is accomplished through a combination of evidence-based communication strategies, helping patients accurately self-measure, and by directing patients and families to resources that support medication adherence and healthy lifestyles.
Learning objectives
- List 6 modifiable lifestyle risk factors related to hypertension
- Describe evidence-based interventions for each risk factor that can improve blood
pressure control - Provide examples of communication strategies that promote culturally, linguistically,
and socially appropriate patient engagement in lifestyle change - Identify resources that can support clinicians, care teams, and patients to achieve
lifestyle change
