04/24/25 @ 11:30 am 12:30 pm CST

When partnering with patients the goal is to empower patients to control their blood pressure. This is accomplished through a combination of evidence-based communication strategies, helping patients accurately self-measure, and by directing patients and families to resources that support medication adherence and healthy lifestyles.

Learning objectives

  • List 6 modifiable lifestyle risk factors related to hypertension
  • Describe evidence-based interventions for each risk factor that can improve blood
    pressure control
  • Provide examples of communication strategies that promote culturally, linguistically,
and socially appropriate patient engagement in lifestyle change
  • Identify resources that can support clinicians, care teams, and patients to achieve
    lifestyle change
