Target: BP Webinar – The Importance of Treating Your Patients’ High Blood Pressure

This one-hour live webinar will train physicians and clinical teams on the “Act Rapidly” component of the M.A.P. Framework. M.A.P. stands for Measure Accurately, Act Rapidly, and Partner with Patients, Families and Communities. Acting rapidly to manage high blood pressure is a key component to Target: BP’s ability to improve blood pressure control. This is accomplished by using a combination of: an evidence-based treatment protocol, having patients return frequently for follow-up until their blood pressure is controlled, single-pill combination therapy and incorporating feedback metrics whenever possible to drive improvement for the care of patients.
